Course Evaluation

Hello:
You are invited to participate in our evaluation of Fern Harris teaching techniques and styles in a course or tutoring you participated in. Some of the courses she has taught are: tithes and offering, 10 minutes of music, college level math, GED preparation, masters level business assistance. Please indicate the name of the course you participated in and an approximate date of the course. It will take approximately 5 minutes to complete the questionnaire.

Your participation in this study is voluntary, yet necessary to insure improvements to Fern's teaching style. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your viewpoints.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
